Here’s what the new Form 4564 looks like. The check boxes are not consistent from one case to the next, for the ones we have received. All of them have come from Philadelphia. All three are from one person, whose name I have deleted. The phone number is the generic information hotline that they’ve been maintaining all along, +1 (215) 516-4777, so I don’t know why I bothered deleting it. A phone call goes to voice mail anyway so don’t bother. I’m doing everything in writing until and unless I get a direct dial phone number from someone.

Filed voluntary disclosure on offshore account prior to October 15, 2009. However, failed to file subsequent disclosure due January 15, 2010. Haven’t heard anything from IRS.

I’m not sure what happened with your application. Nor (of course) do I know the facts of your case. But this sounds like you applied for the Voluntary Disclosure Program before the deadline of October 15, 2009. Then you should have filled in the optional format letter to Criminal Investigations with the details of your case. In short order you would be the happy recipient of a letter from Criminal Investigations saying they have no interest in you.
If this sounds right — you didn’t file the optional format letter specified in Q & A #6 from the IRS — then I think you need to get yourself in front of an experienced person who can tell you what the situation looks like for you and what your next step should be.
